Transition Plan

Transition Plans, sometimes called “Accessibility Plans” or “Improvement Plans” are created by organizations after conducting an accessibility self-assessment to develop a plan for removal of barriers for people with disabilities. A transition plan contains a list of the barriers that limit the accessibility of programs, activities, or services, the ways the organization plans to remove those barriers, the schedule or timeline for implementation, and the person or team who will be responsible. Transition Plans are required under Title II of the Americans with Disabilities Act (ADA) and the Rehabilitation Act, and strongly recommended for Title III organizations, and should be updated periodically.
